3.      Briefly describe what is meant by the "extended organisation".  In particular, how does the way successful organisations today view their suppliers (and their suppliers ...) and their customers (and their customers ...) as opposed to how organisations viewed them in the past.

The extended organisation relates to the fact that today businesses don't see their business stopping at the physical boundaries of their business and don't only think about their direct suppliers and their direct customers.
Successful businesses today think of their business as being a part of the entire supply chain and try to understand and work with all the businesses across the supply chain.  In particular though, they share information across the supply chain.
An example of this is that a supplier to a business could help the business in the design or development of their product or service (or at least the part relevant to that supplier). Similarly, customers of the business could help with marketing and sales plan for the business (to customers downstream).

4.      A Supply Chain Specialist in setting up a supply chain aims to:
Maximise efficiency in some aspects of the supply chain and maximise effectiveness in others (or at least find an appropriate balance of effectiveness and efficiency).
